Attention Portal Users: Important Information About Changes to Our Patient Portal
We are pleased to announce that we will begin enrolling patients in our new Patient Portal powered by NextGen® starting on Wednesday, February 15th. Invitations to enroll in our Portal will be sent out to patients enrolled in our previous Portal via email over the next several weeks. The new Portal will provide an easier and more convenient online experience for our patients with a simpler enrollment process. Please visit our Portal News page for more information.
Effective December 12, 2022:
- Request Prescription Refills – While the new Patient Portal is being prepared, you’ll still be able to request prescription refills through our website which can be found by visiting our Appointments & Rx page and clicking the Prescription Refills button.
- Provider Communication – If you need to communicate with your providers, you can still do so by visiting our Appointments & Rx page and clicking the Email Your Doctor button.
- Medical Records – Your current Patient Portal (NextMD) has been switched over to read-only mode. You’ll still be able to view, download, and print your historical medical records; however, any medical records requests for office visits or tests occurring on or after 12/9/22 must be submitted via the Interim Portal Medical Records Request Form on our website.

ATTENTION PATIENTS: Please be informed that the Interim Portal Medical Records Request Form is only intended to be used TEMPORARILY starting on December 9, 2022, until the launch of the new portal in early 2023. Please fill out the request form if you require records during this time range ONLY and please allow 3-5 business days for processing.

Note- Our normal Request for Medical Records process will still be in effect: if you have specific visit dates or need medical records from visits prior to 12/9/22, please visit our Medical Records page to complete the Request for Patient Access to their PHI form per our normal request process.
