Stephen A. Klein, MD, to Provide Primary Care Out of New Location in Suffern
West Nyack, N.Y. – August 3, 2017 – Effective August 14th, 2017, Crystal Run Healthcare’s primary care office and providers located at 200 Route 59 in Suffern will be relocating to the Practice’s newly opened office located at 156 Route 59, Suite C-5 in Suffern, NY. Primary Care providers, Stephen A. Klein, MD and Leslie Cooper, MS, NP-C will be available to see patients in the new location during the following office hours:
- Monday through Thursday, 8:00am – 7:00pm
- Friday, 8:00am – 5:00pm
- Every Saturday, 9:00am – 2:00pm
For current patients of Dr. Klein’s or Ms. Cooper’s no action is required. All medical records will remain with him in the new office location. For any questions or to schedule an appointment, please call 845.369.9300.
