Facility Obtains Highest Achievable Level from the National Committee for Quality Assurance (NCQA)
West Nyack, N.Y. – May 25th, 2017 – Crystal Run Healthcare’s state-of-the-art medical facility in West Nyack was recently granted Level 3 Patient-Centered Medical Home (PCMH) recognition by the National Committee for Quality Assurance (NCQA) – the highest achievable level of recognition that a patient-centered medical home can receive.
The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care, and reduce costs. Medical homes foster ongoing partnerships between patients and their personal clinicians, instead of approaching care as the sum of episodic office visits. In receiving Level 3 Patient-Centered Medical Home recognition, the NCQA has verified that Crystal Run has made a commitment to providing quality improvement within its practice and employs a patient-centered approach to care, resulting in happier and healthier patients.
“Crystal Run Healthcare believes in NCQA’s Patient Centered Medical Home model because it aligns with the mission of the practice, namely to improve clinical quality, care coordination, and the patient experience. We are thrilled that our West Nyack site has obtained the highest level of recognition from NCQA and applaud the effort of all the physicians, nurses and staff in West Nyack for their work to bring this model of care to Rockland County,” said Scott T. Hines, MD, Chief Quality Officer, Medical Director, at Crystal Run Healthcare.
The NCQA’s Patient-Centered Medical Home Recognition Program is the most broadly endorsed PCMH evaluation program in the country, with more than 12,000 practices (with more than 60,000 clinicians) recognized.
