Facility Obtains Highest Achievable Level from the National Committee for Quality Assurance (NCQA)
West Nyack, N.Y. – November 20, 2017 – Crystal Run Healthcare’s pediatric office in Haverstraw was recently granted Level 3 Patient-Centered Medical Home (PCMH) recognition by the National Committee for Quality Assurance (NCQA) – the highest achievable level of recognition that a patient-centered medical home can receive.
The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care, and reduce costs. Medical homes foster ongoing partnerships between patients and their personal clinicians, instead of approaching care as the sum of episodic office visits. In receiving Level 3 Patient-Centered Medical Home recognition, the NCQA has verified that Crystal Run has made a commitment to providing quality improvement within its practice and employs a patient-centered approach to care, resulting in happier and healthier patients.
